2016 KW T-680 ISX15 CM2350 -Code 125- At Wit's End...
Obtained tractor Jan 2020 @ 390k mi.
At this time, tractor @ 690k mi.
No major issues since taking ownership prior to May of this year. Would like to keep tractor mandated. I don't idle my tractor. I use my APU & bunk heater.
Oil changes every 18k-20k miles. Lab-tested oil samples every oil change for the past 2 years. No problems detected. DOC/DPf filters cleaned twice since Jan 2020. Last May, had Di@sel F@rce treatment done at shop in Butler, PA. I don't run hard speeds OTR, prolly average 63-65 mph. Since visit to Butler, PA shop, started using MM at every fuel up. My last overhead was done a couple months ago. My tractor has been serviced with all new filters since this problem originated and double checked. Their replacement has seemed to not have any affect on this behavior.

Started noticing what i thought was excessive passive regen with tractor starting about 4 months or so ago (this past May). Prior to overhead being done. In June, took tractor to shop to have checked with Insight. At that time, tractor still hadn't and wasn't throwing any codes. But, I just felt my tractor wasn't acting "normal". Shop performed forced regen. No anomalies found. No codes thrown.
Left shop and drove tractor OTR approx. another month.
In July, tractor throws CEL 2754. Tractor goes onto C#mm!ns shop floor. Tech explains 10 trouble shooting steps required for that code. I get my tractor back the next day and Tech is unable to duplicate that code or find any issues other than DPF Analyzer Test-Fail. DPF Restriction Test-Passed. Tells me Regen looked good, as far as Temps, but I may need new DPF/DOC. Tech noticed DPF inlet temp slightly lower than DPF outlet temp. Tractor has no codes.

I leave there, drive approx. 325mi to P@terb!let. No codes on tractor. Still noticing what seems like passive regen behavior from my tractor along the way.

I show up at this shop now the next morning with no codes, but explain my issue (thinking I may have some face-plugging on my DOC) and ask them to pull my DPF/DOC filters and clean them.

That evening I get my truck back. Am told everything went well and no problems found with filters.

I leave that shop.

Approx. 1 month later, my tractor now starts throwing Codes 2754, 125, 2639, & 1921. This time, my tractor goes into K£nw@rth shop floor. Now, I'm told I need a new Turbo/Actuator & DPF filter.

Last weekend I had a new Turbo/Actuator & DPF filter installed on my tractor.

Drove approx. 600 miles only to sense problem(s) not gone away, although tractor still not throwing code(s) again.... yet. Delivered my load, on my way back to get it back into a shop with Insight when tractor throws code(s) 2754 & 125....again. Given 3hr Derate Warning on dash.

Found Rawze on Google which lead me to this forum. Watched his EGR Tuneup video(s). Tractor was back on the shop floor this weekend in Winder where I replaced IMAP, Delta-P, Barro sensor, backpressure sensor. Performed CaC test w/ no leak from CaC. Did find exhaust leak on EGR, and Exhaust manifold. Removed both, cleaned, and resealed with new gaskets. Cleaned 7th injector well and resealed with new gasket(s). Inspected/ cleaned small crossover tube. Still using existing EGR valve. Am currently waiting for new EGR valve to arrive tomorrow.

Drove tractor 48 miles up I-85 this morning. No issues on the way up. Parked up & Shut tractor down approx 20 min. After approx. 20 minutes, Started on my way back, came down access ramp, building speed onto I-85 W.....then, (either between shifting 8th gear to 9th or 9th gear to 10th), felt momentary loss of power and immediately dash illuminated with Derate Warning 1hr. along with CEL which were now know again (Insight) is Code 125. No other codes, just 125.
We've gotten soapy water and sprayed intake manifold, compressor to intake manifold line, turbo intake, charge air intake & hoses, EGR, exhaust manifold... while elevating RPM to approx 1000-1100 RPM. No leaks found. Nothing, nada Frustrating.

At this point, I've spent thousands.

So, tomorrow, the new EGR valve will be installed. I'm at a total loss with what to do next. At this point, all I feel I'm doing is throwing money at the problem.

Seriously, I don't know what to do next.
Any help would be greatly appreciated.

I just wish I'd found this forum sooner.

Just brought the tractor back into the shop floor. While the tractor never quit displaying the CEL 125, the Derate message of 1hr displayed on the dash cluster disappeared. It went away after startup. It wasn't until during the Fuel Injector Test run buy Insight that the Derate message reappeared. After the test concluded, the Derate message, once again, went away.
Am I looking at a Dyno now?

Any help greatly appreciated.

Ran a Hypertensive Fuel Injector Test thru Insight.
All 6 injectors passed all 4 test points.

Monitored EGR Cooler Efficiency with Insight while tractor at resting idle @ 600 RPM and at elevated idle @ 1100 RPM.

EGR Cooler Efficiency stayed flat at 0% across the board.

EGR Temp averaged 244°F at engine operating temp.

Intake Manifold Pressure @ idle was 0.0 inhg
With a maximum of 3.0 inhg @ 1100 RPM
