I've actually seen cams quite a bit worse than the one Rawze posted a pic of.
-The best maintainence you can do for them, shim the oil pump to create more oil pressure, which results in superior protection IMHO. A fair amount of us on here have done this. We do not have years and years, with millions of miles on many engines to scientifically prove this theory, but no one has disproved it either.
-Frequent overhead adjustments reduce the risk of valve(s) wearing enough to eliminate rocker clearance causing the rollers to ride the cam continuously not allowing the oil to get where it needs to be. I now do them annually.
-Frequent visual inspections will catch issues early, possibly limiting you to either a cam going bad, but the rocker rollers not when you spot it, or vice versa. Popping a rocker cover is so simple and requires such a small time investment, I do this quarterly.

When you are inspecting a potential used truck for purchase, not popping the rocker cover is inviting disaster. I have been pointed to some heavy equipment forums discussing cam issues in Cats as well. There are many anecdotes on this site of other engine mfg's with cam issues as well, it's a gamble I would simply not take.
