Now I am wondering if it’s a slack adjuster issue. Perhaps it’s binding internally preventing it from backing the s cam off completely?

Something has changed between the last time you did brakes and now. The spider looks to be fine from the pics. The only thing I can think of is a alignment issue of the cam. Which leads me to think the slack adjuster may be the culprit.
