Spin it with the belt off. If the bearings are shot you'll hear a grinding noise or see a wobble to the fan. Also grab the Hub and wiggle it if there's excessive movement then the bearings are shot or the insides are worn out. If it's the Hub itself that's worn out just get a reman'ed or new hub. If it's the pulley hub bearings that are shot take it off the truck take the nut off, pull the snap ring off and tap the pulley off of the Hub. Those bearings are easy to replace. Just pound them out with a hammer and punch. For reinstalling watch how Rawze's does his wheel seal bearings on his YouTube video. You can do the same exact thing and it's highly recommended you heat the pulley and freeze the bearings to install. The pulley hub is made of a surprisingly soft Steel and won't take much of a hammer whack to mess it up.

As far as finding the pulley hub bearings just take the digit numbers and call up any bearing parts store and they should have a couple different choices for you to replace them. Oftentimes you won't find the exact manufacturer and part # of the bearing that came in the hub.
