Do I need an overhaul?

My truck runs at 40 psi oil pressure pretty much all the time. It doesn't burn any oil that I know of but it does leak some. We've discussed me needing major work on here but I really wanna try and pinpoint how much longer I can go. Also it doesn't run very hot and it think that really helps the oil pressure stay up at 38-40. Usually it runs 160-190 with the occasional spike at 210 loaded and pulling. At a full 210 at the end of OCI it drops to 34-38 psi but on a fresh change it's always 38-40. I do have a fair bit of blowby but the truck runs really well. 1.2 million miles with no overhaul that I'm aware of but I've always wondered if maybe it was inframed before we got it. Everyone here says I should just stay on top of maintenance instead of trying to trade it for something else. I really wanna get as much insight as I can here and see what the next step is. The biggest question I have is should I be afraid of engine failure where I am? As always I'm here to learn.
