Typically you replace the guts as a precaution BEFORE they fail, also performing the other tests to the head (If I remember off the top of my head there's a check valve inside the head that isn't replaceable if it fails). Usually after a failure there's damage to the entire pump, not just the head, and of course the rest of your engine.
