most common cause is clogged egr piping (or egr cooler), poorly reading egr/engine sensors, and then turbo is being commanded to higher than normal positions/over-spooling. Bad connections, leaky CAC or exhaust systems.. you name it .. the engine is going to react to it.

it can also be that some moron installed the wrong program for the engine (in attempts to up the HP). Does the program in the ecm match exactly, what the engine ser# calls for on cummins wwwsite?.

read my post above and do ALL of the things recommended 100% and don't give it any excuse to act up.
