Truck well maintained, should I even check further or move on. Obvious noob here

I've got a buddy with a similar spec 10 speed eaton.
I would stay 10 feet away, buddy always complains about No power and mid 6's fuel mileage!!

I've heard from a couple guys that they do real good when tuned.
