The TSB (Technical Service Bulletin) is TSB150147
Part number is 3691476 for the maintenance-free breather insert.
It fits 871's, 2250's, and 2350's.

Just sharing my latest situation, no intention to convince anyone that i did the right thing. After my truck started crankcase pressure issue it had 620k miles, checked it with Cummins dealer and a few mechanics and all said it was headgasket. since i had another engine of the same specs, i decided to run the truck as much as i can, i drove it to 854k using block sealant (don't want to mention name), truck did blow a coolant reservoir and a hose, blocked sleeper bunk heater during this period. She is in shop for engine swap now, will update photos of how engine looks like from inside once i get it opened. It worked good for me, drove the truck for a bonus 230k miles on a damaged head gasket.

RE: crank case pressure
I've come to know something strange about my 2009 Volvo with CM 2250 that might help someone. My truck would sometimes lose the throttle down the road (would go to idle speed from 60/65mph), easing up on gas pedal would bring the juice back, insite showed code 3528, 3545 and 3326 indicated a bad accelerator sensor (gas pedal) or wire issue between accelerator sensor and ECM. i got the gas pedal replaced ($180.00 plus labor) but this did't do any good. Next day truck brought this symbol on dash (Photo Attached), I replaced the cranckcase filter and that was it. All codes gone and truck running just normal.

I've come to know something strange about your 2009 Volvo as well, and this may help you...
If the engines in your pictures are from your truck(s), including the salvage one, neither engine is a CM 2250. Both engines in the pictures in your post are most assuredly CM871's.
CM2250's are common injector rail engines, and DO NOT have IFSM's on them. Also CM871 EGR crossover pipes go around the back of the engine right above the flywheel housing, like in your pictures, and CM2250 EGR crossovers go over the rocker cover, unlike your photos.
Just thought you should know.

RE: crank case pressure
I ran Rawze's manometer test on my3/2013 model 2350 isx 600 460,000 miles and at idle the level of separation was 5" and at 1500 rpms it went down to 2.5". That being said, I have plenty of power and no noticable problems other than oil consumption ,3 gallons in 10,000, and the smell of burning engine oil when coming to a stop. No exterior oil leaks. My next step is to get an oil analysis. I still think this is an odd result. Any ideas ( If this is the wrong thread for this please Redirect me, thanks).

CenPen777 sounds more like piston rings than anything else.
