Everything your describe screams a lifetime of neglect and also highly likely the thing has been driven its whole life mostly below 1500 rpm (considered abuse) . - Sad indeed....

We bought it at 350k and I have been the driver since purchase. I religiously follow your advice. Egr tune up when we got it, except egr cooler. Oil changes at 10-12k. New doc and dpf. I’m really ocd and picky.
When I get back to the shop I’m personally changing the egr cooler and turbo. Probably get some rail caps and start testing injectors.
I can basically spend and do as I need. Wish we had freight to Georgia.... not giving up on it yet. Still a nice truck.
