Start with a full EGR Tune-up on it...

== When is the last time the IMAP and EGR sensors were replaced?. .. How about the EGR Delta-P sensor replaced and its mounting block passages cleaned out?. How about the EGR pressure sensor, and all the others?.

== When is the last time the EGR piping was removed and cleaned out?. .. The EGR valve?.

== When is the last time the Exhaust pressure sensor was replaced?... and its circuit checked to ensure it is not clogged up?. The passage going thru the thermostat housing is notorious for getting clogged up.

I.E.... cleaned out 100%. - A regen cycle (parked or otherwise) on the truck is not capable of cleaning the DPF by more than about 2-4% at best ... so this also has to be done about every 2 years for a healthy system.

--=ALL OF THOSE THINGS mentioned above should be done regardless of what you suspect.. ALL OF THEM .. at least once every couple years/250,000 miles or so. Its called TUNE-UP WORK.. and no repair shop is going to any of it at all. It is solely up to the truck owner to keep after this type of preventative maintenance and no one does this until all they have is problems. - Welcome to the world of modern trucking.

It can also be that the fuel rail is getting leaky, or some other system not in good health on the engine, and the ecm simply being in a different operating mode is detecting something wrong but not enough to throw a fault yet for it.
