Remove it, clean it thoroughly with carb cleaner, use some penetrating lubricant ONLY if required, while attempting to free it's movement, if it moves freely full travel after then no. Just clean out penetrating lubricant as the soot will love to stick to that, and reinstall.
Test the actuator as well while removed to ensure it travels properly.
If you can't free it's movement, or the spring is broken or damaged, then yes it must be replaced.

It can also be the delta-P sensor bad, or clogged crossover tubes. It can also be other problems/sensor issues that make the engine want more EGR than the valve can give.
A thorough EGR tune-up and cleaning out the EGR valve and testing it is a good start.

It can also be the wrong software in your engine causing it if someone put a different HP setting (calibration) in.
